These form is managed through the offices of the Graduate Coordinator and Academic Programs. In Year 1, students are required to submit the form to Synergy Gateway. In Year 2, students will be uploading the form as directed by the program. Throughout the two-year program, placement sites may request this form multiple times. Students are therefore advised to make several copies in advance and keep them readily available. The original form must be submitted to the University.
