Below are two sign-up forms for a variety of clinical teaching opportunities.  We are so grateful for the ongoing contributions of speech-language pathologists to the education of our students.  By completing this survey, you will be indicating your commitment to participate in one or more clinical teaching activities.  You will receive a confirmation e-mail and, closer to the time of the commitment, the appropriate staff or faculty member will follow–up with you to make specific arrangements.
